GENERAL INFORMATION
In the Law No. 6698 on the Protection of Personal Data (“PDP Law”), personal data owners (hereinafter referred to as the “Applicant”), who are defined as the data subject, have the right to make specific requests concerning the processing of their personal data pursuant to Article 11 of the PDP Law.
As stipulated in the first paragraph of Article 13 of the PDP Law, applications related to these rights, to be directed to our Company, the data controller, must be submitted in writing or through other means as determined by the Personal Data Protection Board (“Board”).
In light of these provisions, applications to our Company can be made by printing out this form:

- and submitting it through a Notary Public.
- sending it to the Company’s registered e-mail address by signing it with the “secure electronic signature” defined in Electronic Signature Law No. 5070.
- and sending it by using the mobile signature method.
- and sending it to us through the e-mail address you have previously provided to our Company and registered within our Company’s systems, if applicable.
Below, we provide information on how written applications should be delivered to us, specific to the respective channels:

In accordance with Article 13, paragraph 2 of the PDP Law, we will respond to the applications you submit to us within 30 (thirty) days from the date of receiving your request, depending on the nature of the request. Our responses will be conveyed to you in either written or electronic format, as specified in Article 13 of the relevant PDP Law.

To mitigate legal risks stemming from unauthorized or improper data sharing, especially to safeguard your personal data, our Company reserves the right to request additional documentation and information (e.g., a copy of your identity card or driver’s license) for identification and authorization. If the information related to your requests within the scope of this form is inaccurate or outdated, or if an unauthorized application is made, our Company disclaims any liability for requests arising from such inaccuracies or unauthorized applications.
